We are accepting new clients.

We ask clients to come about 15 minutes early for the first appointment to fill out necessary forms and review the notice of privacy practices.  However, if  you prefer to review and fill out the forms before coming to our office, you may download the forms for your therapist below. The intake forms include our "Notice of Privacy Practices" for clients.
